What is it?

A form that asks permission to take young people on trips. This allows for the parents/guardians to be informed for an ‘out of the normal’ event, and to give permission for the young person to take part.

This form should contain

• Name and details of young person
• Details of the event; where, where and what’s involved
• Parent and emergency contact details
• Transport details
• Parental consent

Retention time

Six years.
